The Ontario Minor Hockey Association is continually striving to improve our programs for our membership. We value your thoughts and opinions on your participation in the OMHA during the 2024-25 season.

The objective of this survey is to obtain information from players and families to gain a better understanding of your experience and to identify areas for improvement.
